When do I need a fit note?

How to request a Sick (Fit) note if you have been off work for more than 7 days

To request a sick note, please fill out an admin request using our patient triage form.

If you are unable to complete the form online, please contact the surgery on 01442 263511 and a member of staff will complete the form for you. 

Sickness for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification. Click here for information and the form for self-certification

More than 7 days off sick

If you’re off work sick for more than 7 continuous days (this includes weekends), your employer will usually ask for a fit note (or Statement of Fitness for Work) from a GP or hospital doctor. Fit notes are sometimes referred to as medical statements, sick notes or a doctor’s note. More Information

Fit notes cannot be issued before their start date for example: if you require a note to start on the 2nd July, we cannot issue until this date.

Fit notes can be back dated, if needed.

There maybe a charge if you require a private certificate/letter for occupational purposes.
